This application allows us to get to know you and learn why you are interested in purchasing and owning a Chamonix Toy poodle. The length of this application may intimidate you, but please keep in mind that I am trying to ensure my "children" go into what I feel is the best possible home. Submitting this form does not obligate you to purchase a puppy nor does it guarantee that we will have a puppy for you. However, this application will place you on my puppy waiting list if I approve you for one of my babies. If you do not wish to be placed on the list please let me know in the "additional comments" box at the bottom of this form!
